Library Card Application

All residents of Ohio, age five years or older, may obtain free registration for borrowing library materials. Patrons under 14 years of age must have a parent or guardian sign the application card. A picture ID and a document showing current residence are required at the time of registration. Patrons may borrow up to 15 library materials at a time. Households may borrow up to 4 DVDs at a time.

Loan Periods

  • Books & Magazines.......................................2 weeks
  • Music CDs.....................................................2 weeks
  • Teachers (up to 25 items).............30, 45, or 60 days
  • DVDs.......................................................7 days or 14 days for seasons

     

Other Borrowing Policies

Patrons may renew library materials over the phone or in person one time. New materials can be renewed if not on reserve. Renewals are not permitted on DVDs.

Movies may be checked out on your own card if you are 14 years or older. Materials with an "R" rating will be limited to those 18 years or older.

For your protection and ours, it is the policy of the library that no patron is permitted to borrow items on another patron's account. Only two exceptions exist for this policy:

  1. Materials for youth under 5 years of age are placed on the parent/guardian's account.
  2. With a one time written parental permission, youth under 14 years of age may borrow DVDs on their parent/guardian's account.
