The Golden Buckeye Card is available to Ohio residents age 60 and over who hold a current Ohio Driver Licenses or State ID, as well as disabled adults ages 18-59. The program provides discounts at participating local businesses for members.
Ohio residents with a state ID or driver's license should automatically receive a card in the mail on their 60th birthday. If your Golden Buckeye card has been lost, stolen, or damaged or you never received your card, call 1-800-422-1976 or fill out this online form to request a replacement.
If you'd like to submit a paper application for your card, print out the form below or pick up a copy at the library. A staff member can help you fill it out. To complete the application you must bring your ID, showing proof of age and address.
